Service Inquiry and Referrals

We understand the importance of accessing mental health supports in a timely manner and we are working hard to keep wait times down. If you would like to make a service inquiry or referral, please email us at info@pmhc.ca to request a referral form. Please include a phone number in your email to ensure we can follow up with you if required. If we are able to take on new referrals at the time of your request, we will provide you with a referral form to complete and submit.
